长期从事骨骼肌肉系统疾病临床基础转化研究,近年来大力推进临床研究规范化管理

现为清华大学长聘教授,北京清华长庚医院临床研究中心主任;国家级高层次人才计划入选者、国家特聘专家;澳大利亚悉尼大学荣誉教授、塔斯马尼亚大学兼职教授。 在安徽医科大学获医学学士(1990)、医学硕士(1993)和医学博士(1999)学位。1999年底于安徽医科大学破格提拔为教授;2014年在澳洲塔斯马尼亚大学晋升为教授;2018年至2025年担任南方医科大学教授、珠江医院临床研究中心执行主任。曾任莫纳什大学兼职教授、塔斯马尼亚大学全球参与委员会委员、孟席斯医学研究院副院长等。为澳大利亚政府ARC Future Fellowship、澳大利亚政府国立卫生和医学研究院(NHMRC) Career Development Award获得者。 在JAMA、JAMA Internal Medicine、Science Translational Medicine、Annals of the Rheumatic Diseases、Arthritis & Rheumatology、Radiology、Nature Reviews Disease Primers等国际杂志发表论文380余篇,第一和通讯作者SCI论文共230余篇。被引1.9万余次,H指数为75(Google Scholar,2025年底数据)。连续多年入选中国高被引学者,入选全球前2%顶尖科学家终身科学影响力排行榜(关节炎和风湿病领域位列国内首位),入选ScholarGPS骨关节炎领域全球前0.5%顶尖学者(中国排名第一)。 承担澳大利亚NHMRC基金、中国国家重点研发计划、国家自然科学基金等30多项课题。研究结果多次被路透社、福克斯、雅虎、澳大利亚广播公司、中国新闻网等媒体所报道;多次受邀在国际、国内学术会议上大会发言。 主编《研究者发起的临床研究研究者手册》(人民卫生出版社);主导制定20项广东省“研究者发起的临床研究规范化管理共识”,并推广到全国。

  • 围绕骨关节炎这一重大疑难疾病,开展系列临床研究、基础和转化研究,探讨其危险因素、发病机制、分期分型、早期诊断及治疗措施。

    1. 开展维生素D在膝骨关节炎发病机制中作用及维生素D补充治疗膝骨关节炎的研究:在队列研究中发现维生素D缺乏与关节腔狭窄和两年后的关节软骨损失成正相关,与五年后关节疼痛成正相关,提示维生素D补充可能防治骨关节炎;临床试验发现膝骨关节炎患者补充维生素D两年虽然不能明显减少关节软骨损失及一常规方法(WOMAC)测定的关节疼痛,但是能明显改善关节功能和另一方法(VAS)测定的关节疼痛,对软骨下骨髓损害有一定作用;事后分析发现维生素D补充两年能明显减少关节腔渗液性滑膜炎、抑郁症状和足痛。因此,维生素D补充对膝骨关节炎可能有一定作用。以上成果已发表于国际顶级杂志JAMA及Annals of the Rheumatic Diseases等知名期刊上。

    2. 开展炎症机制在膝骨关节炎中的作用及抗炎药物对炎症性骨关节炎的治疗作用研究:在队列研究中发现血液中炎症因子包括白细胞介素6和肿瘤坏死因子-α水平与膝关节腔狭窄和两年后的关节软骨损失成正比、与五年后关节疼痛成正比。血清炎症标记物C反应蛋白在膝骨关节炎病人中明显升高,与关节疼痛和功能损失相关,与软骨下骨髓损害明显正相关。开展姜黄提取物治疗滑膜炎型骨关节炎的随机对照临床试验,发现姜黄提取物治疗3个月可缓解膝骨关节炎疼痛;开展甲氨蝶呤治疗滑膜炎型膝骨关节炎渗液性的随机对照临床试验,发现甲氨蝶呤治疗1年对炎症性膝骨关节炎疼痛和渗液性滑膜炎的治疗作用与安慰剂相当。以上成果已发表于Annals of the Rheumatic Diseases、Annals of Internal Medicine、JAMA Internal Medicine等国际知名期刊上。

    3. 开展髌骨下脂肪垫在膝骨关节炎中的作用及针对髌骨下脂肪垫治疗膝关节炎的研究:在队列研究中首次报道髌骨下脂肪垫在正常情况下对膝骨关节炎的疼痛和关节结构损害有保护作用,但在有MRI-检测的信号强度改变的情况下关节疼痛和结构损害明显增加;首次利用纹理分析技术对异常信号改变的髌骨下脂肪垫进行量化评分,成功构建髌骨下脂肪垫-3D纹理评分体系,并证实量化的髌骨下脂肪垫纹理特征可显著区分并预测膝骨关节炎的发生。利用单细胞RNA测序的方法解析了髌骨下脂肪垫的细胞图谱及其在膝OA进展中的变化,发现了炎性巨噬细胞和成纤维细胞在髌骨下脂肪垫组织炎症和纤维化中的重要作用,其高表达APOE,促进关节软骨细胞退变。开展超声引导髌骨下脂肪垫内注射糖皮质激素治疗炎症性膝骨关节炎的随机对照临床试验,发现此治疗方式对炎症性膝骨关节炎可能有一定疗效。开展膝骨关节炎患者全膝关节置换术中髌骨下脂肪垫保留vs.切除的术后临床疗效观察多中心随机对照临床试验,发现髌骨下脂肪垫保留vs.切除对术后1年关节功能无明显影响,但对可导致髌骨下移。以上成果已发表于Annals of the Rheumatic Diseases、Radiology、Science Translational Medicine、JAMA Network Open、NEJM Evidence等知名期刊上。

    4. 开展代谢机制在膝骨关节炎中的作用及二甲双胍等治疗代谢性膝骨关节炎的研究:在队列研究中发现血液中瘦素(由脂肪细胞产生的脂肪因子)水平与膝关节软骨容积成负相关,与膝关节软骨厚度损失及与髋关节关节腔狭窄成正相关,并在肥胖与关节软骨的关联中起介导作用。2 型糖尿病患者使用二甲双胍与未使用二甲双胍比较,12年内全关节置换术的发生风险减少30%。正在开展二甲双胍与安慰剂比较治疗膝骨关节炎疗效的随机对照临床试验。以上成果已发表于Annals of the Rheumatic Diseases、Canadian Medical Association Journal 等知名期刊上。

    5. 开展医工交叉背景下基于人工智能的骨关节炎预后研究:在大规模前瞻性队列中,基于膝关节及大腿MRI影像,融合影像组学与深度学习等人工智能技术,自动提取软骨、软骨下骨、半月板、肌肉等多组织高维影像特征,构建骨关节炎发生、疼痛进展及全膝关节置换术风险的预测模型。研究发现,人工智能模型较传统影像学指标在预测骨关节炎发生及临床结局方面具有更高的预测效能,可提前识别高风险人群;多结构影像特征融合模型在预测性能上显著优于单一结构模型,展现出更强的风险分层与早期预测能力;同时表明肌肉质量相关影像特征在骨关节炎发生与进展预测中的重要价值,为骨关节炎的预后评估与精准干预提供了新的技术路径。此外,针对炎症性膝骨关节炎,整合滑膜炎相关多组学数据构建机器学习模型,实现疾病精准识别与分型,为个体化抗炎治疗提供技术支撑。以上成果已发表于Arthritis & Rheumatology、Advanced Science、PLOS Medicine、Osteoarthritis and Cartilage等知名期刊上。

    6. 开展骨关节炎代谢炎症相关机制和生物工程技术在骨关节炎中应用的基础转化研究:揭示肠-关节轴,UDP-葡萄糖脱氢酶(UGDH)乳酸化修饰,非编码RNA包括circNFKB1、lncRNA PILA、miR-214-3p,髌骨下脂肪垫分泌的细胞外囊泡等在骨关节炎发病机制中发挥作用;设计DNA纳米机器靶向逆转线粒体功能障碍、纳米颗粒与靶向肽双重工程等策略增强干细胞效能,从而对骨关节炎动物模型发挥治疗作用。以上成果已发表于Advanced Functional Material、Advanced Science、Biomaterials、Science Singaling等知名期刊上.
